If you fail to request the hearing in the required 15 days, you have WAIVED the hearing and the extension will stand!

1. Contact the MVD
Both of these will be required to start the process:
- Find out the dates or purpose of the extension or suspension
- Find your case/docket number (also listed on the letter you just received)

2. Check your logs online
Check your logs online for violations through your client portal. If they are unavaliable please contact us at 480-945-7028 to requests your logs. Please refer to your MVD Violation Clarification Form or CLICK HERE.
If MVD has a valid reason to give you a suspension/extension but you would like a hearing officer to hear your justification please fill out THIS FORM to request a hearing. However, if you feel that you received this letter in error please continue the steps below.
3. Fill out this form
Fill out the form below in its entirety, if you feel there is an error for the reason why the MVD has given you an extension/suspension.
4. What happens next?
Please allow up to 72 hours for MVD to respond to Safe Harbor or yourself.
