Make sure you have the correct case number that the MVD has given you.
The Reason for Extension is located at the bottom of the letter received, or the reason MVD gave you over the phone.

 


You only have 15 days from the date of notice on the letter SO DON’T WAIT!

f you fail to request the hearing in the required 15 days, you have WAIVED the hearing and the extension will stand!

2. Check your logs online


Check your logs online for violations through your client portal. If they are unavaliable please contact us at 480-945-7028 to requests your logs. Please refer to your MVD Violation Clarification Form or CLICK HERE.

 

If MVD has a valid reason to give you a suspension/extension but you would like a hearing officer to hear your justification please fill out THIS FORM to request a hearing. However, if you feel that you received this letter in error please continue the steps below.
